Clients:  It's optional for your health provider (e.g. doctor) to complete a general referral form or the form below (click on it) and then send it and any other files (e.g. lab results) to me.

Health providers:  You're welcome to send referrals to me.

I would appreciate it if people would share my flyer, poster, or business card with other people.
